Bilateral adnexal cysts are seen measuring about 4 cm each. The left adnexal cyst shows internal dependent high attenuation representing hemorrhage. This cyst is surrounded by high attenuation peritoneal free fluid representing acute clotted blood indicating that this is the site of bleeding (sentinel clot sign). Less dense fluid is seen elsewhere in the peritoneal cavity representing non-clotted blood.

A case of ruptured left ovarian hemorrhagic cyst causing hemoperitoneum showing the sentinel clot sign.
The diagnosis was confirmed on surgery.
