Ruptured intracranial dermoid cyst

Case contributed by Erik Ranschaert
Diagnosis almost certain

Presentation

Sudden and brief loss of consciousness followed by headache.

Patient Data

Age: 85 years
Gender: Male

MRI brain

mri

Lipid-containing (and therefore strongly T1 and T2 (FLAIR) hyperintense ) material is floating on CSF in the anterior horns of the lateral ventricles. Multiple small droplets of T1 hyperintense lipid material can be seen in the subarachnoid space of both hemispheres. The originating (ruptured) cyst difficult to identify but is most probably located in the right parasellar area. There is no convincing solid enhancing component, although fat saturated sequence would be of benefit.

Annotated Images

High T1 signal material is floating on CSF in the anterior horns of the lateral ventricles (blue arrows), and multiple small droplets of T1 hyperintense material can be seen in the subarachnoid space of both hemispheres (yellow arrows).

An area of encephalomalacia (yellow dotted line) is consistent with an old left MCA territory infarct.
