Presentation
Acute pelvic pain for 6 hours. Sudden drop in blood Hb level. The previous ultrasound reported an adnexal cyst.

The right ovary is bulky in size (4.4 x 4.4 x 4.2 cm), showing a shrunken cystic space (3.5 x 2.5 cm).
A moderate amount of blood signal fluid is seen in the pelvis.
The left ovary is normal.
The uterus is normal.
Case Discussion
Considering the clinical presentation and imaging findings, the features are suggestive of a ruptured adnexal with pelvic bleeding.
The patient underwent urgent surgery, and about 1 liter of blood was cleared from the pelvis.
