Saccular common iliac artery aneurysm with retroperitoneal rupture

Presentation

Acute lower abdominal pain. History of blunt abdominal trauma 4 years back.

Patient Data

Age: 78 years
Gender: Male

Large heterogneously hypoechoic retroperitoneal mass with no internal vascularity in left side of abdomen overlying left psoas muscle. 

Left common iliac artery showed presence of a large saccular aneurysm with typical turbulant flow and yin-yang sign on color Doppler.

Left common iliac artery shows a large saccular aneurysm proximal to its bifurcation with partial thrombosis. The aneurysm measures 5.5 cm in its greatest dimension. A large heterogeneous iso to hyperdense collection is seen in retroperitoneum causing superior displacement of right kidney consistent with hematoma.

Moderate left and mild left pleural effusions are seen.
