Abnormal subchondral bone marrow signal involving both surfaces of the sacroiliac joint and displays low signal on T1 and high signal on T2WI and fat-suppressed images with enhancement after contrast injection denotes an ongoing acute inflammatory process
Preserved joint space of both ligamentous and synovial parts
No subchondral sclerosis or cystic changes
Normal shape and signal intensities of the muscle attachments.
Case Discussion
MRI findings are consistent with bilateral acute sacroiliitis.
