Sacroiliitis - Brucella

Diagnosis almost certain

Presentation

Two months history of fever with severe pain of the right buttock radiating to the lower limb.

Patient Data

Age: 25 years
Gender: Male
mri
  • extensive bone marrow edema of both surfaces of the right sacroiliac joint of low signal on T1, high signal on T2 with prominent enhancement on postcontrast sequences

  • joint effusion with edematous infiltration of the ipsilateral erector spinae and piriformis muscles as well as the sacral foramina

  • normal appearance of the left sacroilic joint

Case Discussion

MRI features of a unilateral sacroiliitis. The serum agglutination test was positive for Brucella infection.
