Saddle pulmonary embolism

Extensive pulmonary embolism is seen involving bilateral main pulmonary arteries with extension into lobar and segmental branches. A saddle embolus is seen as well across the main right and left pulmonary arteries.
Right heart strain is noted in the form of straightening of interventricular septum and IV contrast reflux into the intrahepatic IVC and hepatic veins.
Case Discussion
Saddle pulmonary embolism is commonly associated with right heart strain. Leg deep veins thrombosis is a common risk factor.
