Saddle pulmonary embolism

A large filling defect straddles the bifurcation of the pulmonary artery trunk and extends into the left and right pulmonary arteries, inferring saddle pulmonary embolism. In addition, the pulmonary artery trunk is dilated, and straightening of the interventricular septum is noted due to the massive embolism.
In imaged part of the upper abdomen, moderate ascites are evident.
Case Discussion
Large saddle pulmonary embolism can completely obstruct both left and right pulmonary arteries, resulting in right heart failure and death unless treatment is prompt.
