Salter Harris III ankle fracture

A 13 year old boy was brought to the ED with a swollen left ankle upon which he could not weight bear. He had jumped onto an embankment and sustained a dorsiflexion injury.

Patient Data

Age: 13
Gender: Male

There is a vertical fracture in the tibial plafond extending to the articular surface with a 3mm gap and no step. The growth plate is involved laterally. This is a Salter-Harris type 3 fracture. There is soft tissue swelling overlying. No other fracture is demonstrated.

Case Discussion

The patient was offered ORIF due to displacement >2mm
