Presentation
Cough and malaise for 3 weeks. Chest radiograph (not available) showed hilar enlargement. Suspected chest infection, likely tuberculosis.

- multiple enlarged bilateral hilar, subcarinal and right paratracheal lymph nodes
- irregular nodular thickening in peribronchovascular, perilymphatic and subpleural distribution with an upper/mid lung predominance
- some discreet centrilobular small, well-defined, soft tissue attenuating nodules
No evidence of pleural thickening or effusion.
Case Discussion
The distribution pattern of the abnormalities is very likely for sarcoidosis rather than pulmonary infection. No typical features of tuberculosis are present.
