Sarcoidosis associated obstructive jaundice

Diagnosis probable

Presentation

Past history of sarcoidosis. Now jaundice, weight loss and low grade fever. CT showed mass of lymph nodes in root of small bowel mesentery and ultrasound demonstrated dilated intra-hepatic ducts. MRCP for clarification.

Patient Data

Age: 40 years
Gender: Male

Hilar mass lesion causing gross stricturing of the right and left hepatic ducts - looks like a Klatskin cholangiogarcinoma (Bismuth type IV).  Key to the case is the past history of sarcoidosis and finding of abnormal lymph nodes around the aorta and in the small bowel mesentery. The Coronal T2 fat sat demonstrates poorly defined mass in the porta hepatis causing bile duct stricturing and enlarged aorto-caval lymph node

Don't ever forget sarcoidosis in the differential diagnoisis of any mass - it is the great mimicker and can be treated!
