Presentation
Old trauma with scaphoid fracture.
Patient Data
Fractured proximal pole of the scaphoid bone.
Case Discussion
Fractured scaphoid with the proximal pole displaying low T1 signal near the fractured line. However, the presence of some more proximal areas that exhibits fatty marrow signal in T1WI suggest a still viable proximal pole.
In the absence of post contrast images, non contrast T1-weighted MRI would help in the preoperative assessment of the viability of the proximal pole in patients with scaphoid nonunions. STIR images are not the best choice because a high STIR signal is a common findings in patients with and without AVN.
