Scaphoid fracture

Case contributed by Dr Hani Salam

Presentation

History of trauma.

Patient Data

Age: 30 years
Gender: Male
X-ray

Scaphoid views demonstrate a fracture through a lucent lesion at the waist of the scaphoid. No other acute bony injury.

Fracture underwent internal fixation with bone graft and Herbert screw. Note the lucent area in the distal radial metaphysis (site of bone-graft harvest).

CT

Scaphoid fracture through a lucent lesion is again demonstrated.

Case Discussion

Radiographs of the right wrist demonstrated a small lucent lesion of the scaphoid complicated by fracture. The lesion could represent (but isn't limited to) unicameral (solitary) bone cyst, aneurysmal bone cyst or fibrous dysplasia.

CT scan demonstrated the same findings.

Bone graft from distal right radius metaphysis was utilized in the internal fixation of the scaphoid fracture using Herbert screw.
