Scaphoid fracture with dorsal intercalated segment instability

Presentation

Falling on outstretched hand during football game followed by severe pain, swelling and limitation of movement

Patient Data

Age: 21 years old
Gender: Male

There is acute fracture of the scaphoid waist with extensive marrow edema. On the sagittal images the lunate bone is tilted dorsally leading to dorsal intercalated segment instability (DISI)

Case Discussion

Scaphoid fracture most commonly involves the scaphoid waist with possible avascular necrosis of the proximal pole. Carpal instability is a recognized complication of scaphoid fracture with possible VISI, DISI, perilunate, lunate or mid carpal dislocation.
