Scaphoid fracture with dorsal intercalated segment instability

Case contributed by Bahman Rasuli

Presentation

Falling on outstretched hand followed by severe pain, swelling and immobility.

Patient Data

Age: 25
Gender: Male

Acute scaphoid waist fracture with extensive marrow edema is present.

On the sagittal images the lunate bone is tilted dorsally with a scapholunate angle angle >80° leading to dorsal intercalated segment instability (DISI).

Synovial cysts measuring about 6*7 mm and 5*12 mm arising from the triangular fibrocartilage complex and radiocarpal joint respectively.

Case Discussion

Scaphoid fracture account for 70-80% of all carpal bone fractures. Although they occur essentially at any age, adolescents and young adults are most commonly affected.

The scaphoid waist is the most commonly affected site with possible avascular necrosis of the proximal pole. Carpal instability also can be seen as a complication of a scaphoid fracture.
