Cyclical pain at scar site. History of previous Caesarean section.

The study reveals heterogeneous irregularly marginated lesion in right rectus muscle near previous caesarian scar. Right rectus muscle is thickened. Lesion is extending into overlying subcutaneous fat and mildly bulging into pelvic cavity; however, no obvious intraperitoneal extension seen.
Axial and sagittal T2 weighted images reveal heterogeneous irregularly marginated lesion in right rectus muscle near previous caesarian scar.
With a history of cyclical pain and previous Caesarean section this is suggestive of a scar endometriosis. It is thought to be caused by implantation of endometrial stem cells at the surgical site at the time of uterine surgery.
