Presentation

Lower abdominal wall mass sensation is associated with severe pain and tenderness during menstruation.

Patient Data

Age: 45 years
Gender: Female
mri

A relatively well-defined lesion measures about 30 x 45 x 55 mm at the inferior aspect of the rectus abdominis muscles more prominently on the right side.

It displays heterogeneous signal intensity on T1WI and T2WI with internal cystic changes.

There is a 25 x 30 mm left adnexal rounded mass with fat intensity and demonstrating fat suppression suggestive of ovarian dermoid cyst.

Case Discussion

Abdominal wall endometriosis in patients with an abdominal wall mass and classical history of cyclic pain should be considered as a first differential diagnosis.
