Two-year history of nasal obstruction.

A lobulated well-defined soft tissue mass arising from the sphenoid sinus, extending through the sphenoethmoidal recess to the nasopharynx, obstructing both choanae (mainly the left). It shows an intermediate signal intensity on T1WI, inhomogenous high signal intensity on T2WI with marked heterogeneous enhancement following IV contrast.

Case of a Schneiderian papilloma (histopathologically proven).
Schneiderian papillomas, also called sinonasal papillomas, are considered as benign sinonasal tumors, arising from the Schneiderian epithelium of the nasal cavity and paranasal sinuses. Three histological types are described 1-3:
- inverted papilloma: the most common
- exophytic papilloma
- oncocytic papilloma
There is up to 15% risk of malignant transformation to carcinoma (mainly the inverted and oncocytic types).
Treatment of these tumors is primarily surgical.
