Bilateral interstitial pulmonary disease with cystic changes more affecting lung bases.

Bilateral basal fungal balls with air crescent sign

Dilated air filled esophagus down to the cardia

Dilated pulmonary artery (PA = 37 mm) due to cor pulmonale
