Focal right-convex upper thoracic curve with underlying vertebral anomaly T4-6. Large left-convex thoracolumbar curve.

The upper thoracic vertebral abnormality is fixed and structural. To limit thoracolumbar deterioration, anterior spinal fusion across the thoracolumbar junction has been performed.

Case Discussion

Anterior spinal fusion is reserved for complex cases where posterior spinal fusion is not possible. In this case, the upper thoracic curve precludes posterior instrumentation.
