Scrotal hematocele

Diagnosis almost certain

Presentation

Blunt trauma to right side of scrotum 28 days ago. Presents with right hemiscrotal swelling.

Patient Data

Age: 30 years
Gender: Male
ultrasound

Right testis measures: 44 x 25 x 24 mm; vol 14 mL.

Left  testis measures: 44 x 24 x 22 mm; vol 13 mL.

Both the testes are normal in size and echogenicity. No focal lesion is seen. 

Both the left and right epididymides are normal in size and echogenicity. 

Right sided moderate hydrocele with echoes. Septations are noted.

No evidence of hydrocele is seen on left side. 

Both the testes reveal normal vascularity on color Doppler imaging.

Case Discussion

Complex fluid in right tunica sac is likely to be hematocele in view of history of trauma.
