Secondary intracranial hypotension

Diagnosis almost certain

Presentation

Severe postural headaches and nausea started after lumbar puncture.

Patient Data

Age: 35 years
Gender: Female

Pituitary height increased to 9mm . There is effacement of subarachnoid spaces. Moderate sagging of the corpus callosum. There is distention of dural sinuses, especially seen on sagittal images, hypoplastic left transverse sinus is also enlarged. There are no signs of acute ischemia or hemorrhages.

Case Discussion

The patient had severe headaches decreasing on supine position. 

The pituitary was enlarged to 9mm (non-pregnant, non-nursing woman).

Mamillopontine distance is slightly decreased to 6.0mm, but more than 5.5mm.

The most sensitive sign of this condition is the round-shaped transverse sinus on the sagittal plane.

There are no subdural collections findings, which are typical for intracranial hypotension.
