Secondary synovial osteochondromatosis and enchondroma

Case contributed by Dr Matt Skalski

Presentation

Bilateral knee pain.

Patient Data

Age: 65 years old
Gender: Female

There are multiple ossific bodies in the left superapatellar bursa, with mild tricompartmental degenerative changes bilaterally.

The left fibular head demonstrates a well-defined, mildly expansile lytic lesion with cortical thinning and central punctate calcification. There is no evidence of cortical violation or periosteal reaction.

Case Discussion

A classic example of the "2 for 1" show, with two independent and unrelated disease processes seen on one study (however one is much more clinically important). Cases such as this are prone to satisfaction of search, demonstrating the importance of a diligent search pattern.
