Presentation
Pain post twisting injury at football.
Patient Data
Large suprapatellar joint effusion. A curvilinear osseous fragment is projected parallel to the lateral margin of the tibial epiphysis consistent with a Segond fracture.
Further evaluation with MRI is recommended to evaluate for ACL and meniscal injury.
Case Discussion
A Segond fracture is an avulsion fracture from the lateral aspect of the tibial plateau. It is also referred to as the lateral capsular sign. It has a strong association (approximately 75% of cases) with disruption of the anterior cruciate ligament. MRI is essential to define internal injury and plan appropriate treatment.
