Septic arthritis of the hip joint

Diagnosis probable

Presentation

Diabetic patient presented with abdominal pain and distension.

Patient Data

Age: 40 years
Gender: Male

Large multispatial abdominal and right thigh intramuscular collection, showing:

  • inferolateral subcapsular collection around the right lobe of the liver, reaching 9.4 x 5.6 cm in axial dimensions
  • right retroperitoneal region shows enlarged right psoas muscle with intramuscular abscess reaching 7 x 6.45 cm in axial dimensions along its entire length
  • enlarged right iliacus with intramuscular abscess reaching 6 x 8.7 cm in axial dimensions along its entire length. No signs of associated spondylodiscitis
  • right thigh intramuscular extension involving the medial and posterior compartments of the right thigh reaching 11.2 x 11.9 cm in axial dimensions
  • there is associated with moderate subcutaneous edema
  • multiple ipsilateral and external iliac reactive lymph nodes
  • reactive mural thickening of the cecum and distal small bowel
  • mild pelvic collection
  • mild right pleural effusion and bilateral basal pulmonary atelectasis

Marked permeative mixed osteolytic and sclerotic process involving the head of the right femur and right acetabulum with diffuse cortical interruptions and marked right hip effusion, representing chronic right hip joint septic arthritis with hip and acetabular osteomyelitis.

Case Discussion

Right iliopsoas, anterior and posterior compartments of the proximal thigh abscesses with hepatic subcapsular and right thigh intramuscular abscesses, mostly secondary to right hip septic arthritis.
