Septic arthritis of the sacroiliac joint and pubic symphysis

Diagnosis almost certain

Presentation

Two weeks history of pain in the left buttock radiating to the lower limb.

Patient Data

Age: 30 years
Gender: Female

The MRI sequences demonstrate;

  • subchondral bone marrow edema with enhancement of both surfaces of the left sacroiliac joint with joint effusion, and adjacent small abscess formation
  • edematous infiltration of the ipsilateral iliopsoas, obturator internus, and piriformis muscles with swelling and enhancement of the sciatic nerve
  •  enhancement of the pubic symphysis in keeping with septic arthritis with osteomyelitis of the adjacent pubic bones

A small right ovarian cyst is noted.
