Serous cystadenoma of pancreas

Presentation

Abdominal pain with an abnormal ultrasound exam.

Patient Data

Age: 60 years
Gender: Female

The MRI sequences demonstrate a multilobulated cystic mass of the tail of the pancreas with a 'bunch of grape' appearance, not communicating with the pancreatic duct which appears normal in diameter. It is of low signal intensity on T1WI, high signal intensity on T2WI with no enhancement seen on postcontrast sequences. No restricted diffusion is seen on DWI/ADC.

Case Discussion

The MRI features of a probable serous cystadenoma of pancreas.

On imaging, the appearance is similar to intraductal papillary mucinous neoplasm (IPMN) but the absence of communication with the main pancreatic duct is the key to the correct diagnosis.
