Severe hydrocephalus simulating hydranencephaly

Thin cortical mantle is only noted at the gyrus rectus (inferior part of frontal lobe) and temporal lobes. There is also thin rim of brain tissue seen at the frontal region bilaterally and at the lateral aspect of falx cerebri. The falx is present and thick and the posterior fossa is unremarkable.
Case Discussion
Severe hydrocephalus may simulate hydranencephaly. The clue for diagnosis is presence of cortical mantle i.e. rim of brain tissue lining the inner skull table.
