Severe Pott disease of the thoracic spine

Presentation

Worsening neurological deficits including paraplegia.

Patient Data

Age: 25 years
Gender: Male

Paravertebral abscesses (C6 through T10). Multilevel vertebral body destruction, most severe in the upper thoracic spine spanning T1-T5, with a very large prevertebral/paravertebral abscess measuring 2.6 x 4.6 x 10 cm (AP X TR X CC).

The degree of destruction of the T3 vertebral body is most severe, and responsible for retropulsion of bony fragments into the spinal canal with a corresponding epidural abscess bridging the lower T2 through mid T4 epidural space, resulting in severe compression of the spinal canal.

Case Discussion

Tragic case of severe multilevel abscess and vertebral body destruction in a young patient, with characteristic features of Pott disease (abscess spread along anterior longitudinal ligament, multilevel involvement, gibbus deformity).
