Shattered spleen

Road traffic accident victim presented with tense and tender abdomen and deteriorating GC.

Patient Data

Age: 17 years
Gender: Male

The spleen is shattered and there is non-enhancement of spleen on this triple phase scan.

There is extravasation of contrast and a fracture near the hilum.

The splenic vessels are normal.

Large amount of intraperitoneal fluid collection seen suggestive of hemoperitoneum.


Case Discussion

The spleen is the most commonly injured visceral organ in blunt abdominal trauma. A missed splenic injury is one of the most common cause of preventable death in a patient with trauma.

CT is the modality of choice to image haemodynamically stable patients with blunt abdominal trauma with an overall diagnostic accuracy of approximately 95%.
