Shoulder arthrogram (fluoroscopic guided)

Presentation

Pre MRI, for guided injection.

Patient Data

Age: 35 years
Gender: Male
Fluoroscopy

With excellent patient positioning using a foam wedge, the glenohumeral joint is perfectly perpendicular to the x-ray beam.  A 22 gauge Quincke needle cannulates the glenohumeral joint targeting for the posteromedial humeral head.  Injection of iodinated contrast confirms an intra-articular needle tip position.     

Fluoroscopy

The iodinated containing arthrogram mixture is injected subsequently and starts to distend the joint and into the long head of the biceps tendon sheath.  Final image taken after 8mL of the arthrogram solution is injected, the patient then proceeded to MRI.  

Case Discussion

Both anterior and posterior approaches are used to inject the shoulder/ glenohumeral joint.  The MRI arthrogram solution will differ at the different institutes depending on preference and availability.
