Sialolipoma left parotid

Presentation

Incidental swelling in the left parotid. Echogenic mass on ultrasound. Non-tender.

Age: 70
Gender: Female

Lobulated fatty mass in the superficial portion left parotid that suppresses on fat saturation (T2) with no enhancement.  Note minor internal septations

Case Discussion

Sialolipoma is a relatively recent described lesion characterized by mature adipocytes with secondary entrapment of normal salivary elements.  Differential diagnoses are lipoadenoma or fatty pleomorphic adenoma (Fine needle aspiration might be required to differentiate but can be difficult).  Imaging is characterized by a well-circumscribed fatty mass in the gland with minimal solid components.
