Presentation

Abdominal pain.

Patient Data

Age: 80 years
Gender: Male

Segment of thickening, enhancing and irregular proximal sigmoid colon with lobulated intraluminal component and abnormal draining lymph nodes. No liver masses. 

Case Discussion

Confirmed sigmoid colon adenocarcinoma by colonoscopic biopsy. This is a less common appearance which is quite bulky and has a slightly inflammatory appearance. The degree of wall-thickening and enhancement should not be confused with acute or chronic diverticulitis, which can result in thickening but would have less enhancement and more submucosal edema with an associated inflamed diverticulum in the acute setting and less of a bulky/mass-like appearance with intraluminal component in the chronic (muscular hypertrophy) setting.
