Sigmoid carcinoma with colovesical fistula

Diagnosis almost certain

Patient Data

Age: 75 years

Focal, irregular thickening of the sigmoid colon wall is noted with focal fat stranding. An enhancing fistulous connection is visible towards the urinary bladder, which also shows signs of wall thickening on its superior aspect. Presence of a fistula is further confirmed by presence of small gas locules within the bladder. 

Note the presence of extensive diverticulosis, which would make complicated diverticulitis the main differential.
