Sigmoid perforation due to accidental ingestion of toothpick

Presentation

Severe pain with tenderness of the right iliac fossa.

Patient Data

Age: 60 years
Gender: Male

The CT scan demonstrates a linear opaque foreign body (length = 6 cm), piercing the sigmoid wall which is thickened with extraluminal gas, severe perisigmoid fat stranding, and thickening of the adjacent peritoneal reflections.

Case Discussion

CT features most consistent with a foreign body perforating the sigmoid colon with signs of focal peritonitis. At surgery, the foreign body was a toothpick which was confirmed later by the patient that was ingested accidentally a few days ago.

Digestive tract perforation secondary to incidental toothpick ingestion was already described in the literature 1,2.
