Presentation

Diffuse abdominal pain and distension.

Patient Data

Age: 70 years
Gender: Male

Sigmoid volvulus with swirl pool sign or twisting of mesentery resulting in closed loop obstruction. This is associated with mesenteric congestion and edema. Infrarenal abdominal aortic aneurysm down to aortic bifurcation.

Case Discussion

Large bowel volvulus occurs when there is twisting of a colonic segment around its mesenteric attachment. Most common site is the sigmoid colon and tends to occur in old age or in patients with a mental handicap or institutionalization.

Radiological features include massively distended inverted U-shaped loop with proximal colonic dilatation.
