Single live gestation with concurrent intrauterine device

Presentation

The patient presented to Emergency Department with pelvic pain and sporadic vaginal bleeding for five days. The patient had hormonal IUD since 2018. Qualitative beta HCG test was performed in ED and came back positive.

Patient Data

Age: 20 years
Gender: Female

Intrauterine device is identified within the endometrial cavity. Questionable fundal cleft. Single live pregnancy estimated at 11 weeks 0 days gestation.

Case Discussion

Single live pregnancy estimated at 11 weeks 0 days gestation. 

Sonographic findings raise suspicion for Mullerian duct anomaly of the uterus. IUD may be within the left uterine horn and the live pregnancy may be within the right uterine horn, however, interstitial ectopic pregnancy can not be entirely excluded. Recommended OB/GYN consult and formal OB ultrasound in the antenatal clinic.
