Presentation
Obese child with groin and left thigh pain and a limp over a few weeks.
Patient Data

Widening and irregularity of the left capital femoral physis when compared to the right side.

The Klein line is drawn along the lateral aspect of the femoral neck and should intersect a portion of the lateral capital epiphysis.
On the right, the green line indicates the normal appearance of the Klein line.
On the left, the red line is abnormal and barely intersects the capital epiphysis.

Post-operative appearance. The patient underwent percutaneous internal screw fixation.
Case Discussion
This case illustrates a classic slipped capital femoral epiphysis (SCFE): an overweight or obese male adolescent with vague groin pain either without or after minor trauma and radiographs with widening and irregularity of the capital femoral epiphysis and an abnormal Klein line.
