Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Painless swelling of the posteromedial aspect of the left thigh.

Patient Data

Age: 30 years
Gender: Female

Well-defined lobulated intramuscular mass within the semimembranous muscle composed of numerous dilated serpiginous vascular structures isointense to muscle on T1, hyperintense on T2 and STIR with fluid-fluid levels. The post-contrast dynamic sequences show progressive enhancement with almost fill-in on delayed sequences.

Case Discussion

The MRI features are highly suggestive of soft tissue slow flow venous malformation, commonly known as soft tissue hemangiomas.

According to newer nomenclature (ISSVA classification of vascular anomalies), these lesions are referred to as slow-flow venous malformations.
