SMA syndrome

Diagnosis probable

Presentation

Months of poor appetite, early satiety, nausea, recurrent abdominal pain, and marked weight loss. No past medical history.

Patient Data

Age: 45 years
Gender: Male

The proximal duodenum is dilated and fluid filled up to the D3 segment, until passing posterior to the SMA where it abruptly tapers, without evidence of obstruction. Just proximal to this transition point, there is apparent intraluminal soft tissue density lesion within the D3 segment. Paucity of intra-abdominal fat noted.

Case Discussion

Endoscopy found a normal stomach and duodenum, apart from moderate dilatation. In particular, there was no intraluminal mass lesion. A nasojejunal tube was placed. Conclusion was made of likely SMA syndrome and the patient was commenced on nutritional support in order to facilitate weight gain.
