Small bowel obstruction from incarcerated right inguinal hernia

Abdominal pain.

Patient Data

Age: 90
Gender: Male

Multiple distended loops of small bowel centrally with numerous air-fluid levels in keeping with a small bowl obstruction. Gas and faeces is noted within the rectum and colon, indicating this may be an early or partial obstruction. 

CT confirms small bowel obstruction and identifies the cause as an incarcerated right inguinal hernia containing small bowel. 

Case Discussion

  • small bowel dilatation is considered >3 cm in diameter
  • presence of colonic gas does not exclude a small bowel obstruction
