Small bowel perforation due to ingested chicken bone

Presentation

Abdominal pain, distension, and obstructive symptoms

Patient Data

Age: 65 years
Gender: Female

Evidence of the small bowel obstruction with a transitional zone at mid-ileum is seen. On scrutinized reviewing, an intra-luminal dense structure in favor of a foreign body is seen within the dilated ileal loop on RLQ, before the transitional zone. The foreign body has a linear component that penetrates the bowel wall, with a part on the adjacent mesentery. Few extraluminal air bubbles and mesenteric fatty haziness is visible around the penetrated foreign body. Mild free fluid is visible.

The yellow arrow points to the extra-luminal foreign body and air bubble on the ileal mesentery.

Case Discussion

The patient underwent laparotomy and a chicken bone penetrating the ileum with mesenteric inflammation was found.
