Name the causes for small heart size

Small heart and no subcutaneous fat on lateral chest x ray.

Markedly dilated stomach with food product and contrast with no evidence of bowel obstruction. No bezoar. No CT evidence of pseudocyst or other complications related to pancreatitis.
