Soft tissue hemangioma - knee

Case contributed by Dr Ammar Haouimi

Painless swelling of the right knee with a history of recent trauma to the same side.

Patient Data

Age: 2 years
Gender: Male

There is a well-defined lobulated mass within the medial rectus muscle in contact with the medial patellar retinaculum as well as the medial aspect of the quadriceps tendon. It shows an isosignal on TWI high signal on T2WI with a central hemorrhage of a high signal on T1WI and low signal on T2WI and gradient echo. The postcontrast sequences show an intense and heterogeneous enhancement except the central hemorrhagic area. No restricted diffusion seen on DWI/ADC.

Case Discussion

MRI features suggestive of an intramuscular hemangioma with internal hemorrhage (probably of traumatic origin in this case).
