Soft tissue hemangioma - elbow

Presentation

Swelling of right elbow with attacks of pain.

Patient Data

Age: 15 years
Gender: Female

A well-circumscribed soft tissue lesion is seen posterior to the lower end of the humerus that shows heterogeneous hypoechoic texture and irregular internal vascular channels.

x-ray

Soft tissue opacity posterior to the lower end of humerus with two phleboliths seen.

A well-circumscribed soft tissue lesion is noted at the posteromedial aspect of right elbow joint, with heterogeneous texture of bright T2 and STIR signal, low signal intensity on T1 and shows multiple small cystic areas. Post-contrast enhancement is noted. It is measuring 2.5 x 2.3 x 4 cm in AP, SS and CC dimensions. It abuts the medial aspect of triceps tendon from its myotendinous junction till its insertion. It abuts the posteromedial aspect of lower humerus with no bone changes or surrounding soft tissue changes. The ulnar nerve is stretched over its medial margin, very superficial in location, just under the skin surface.

Case Discussion

Features are of benign soft tissue lesion, in keeping with soft tissue hemangioma (slow flow venous malformation) insinuating between triceps tendon and ulnar nerve.
