Soft tissue lipoma - neck

Presentation

Painless swelling of the left parotid region.

Patient Data

Age: 45 years
Gender: Male
mri

Lobulated well-encapsulated soft tissue mass centered on the left posterior cervical space, displacing the adjacent muscles (digastric, longissimus capitis and sternocleidomastoid) as well as the parotid gland. It elicits a high signal on T1 and T2, attenuates on fat sat sequences with thin internal septations of low signal on T2 sequences. Thin rim of peripheral enhancement is seen on post contrast sequences.

MRI features most consistent with soft tissue lipoma of the neck.

The main differential diagnosis is liposarcoma: usually thick enhanced septa or evidence of local invasion.
