Solitary intradiploic metastasis from lung adenocarcinoma

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Hard swelling of the right parietal region of the skull in a patient treated for lung adenocarcinoma.

Patient Data

Age: 55 years
Gender: Female

Well-circumscribed right parietal intradiploic soft tissue mass measuring 36 x 33 x 25 mm with destruction of both inner and outer tables of the skull. It elicits a low signal on T1, heterogeneous signal on T2 and FLAIR with restricted diffusion and heterogeneous enhancement on postcontrast sequences. Thickening with the enhancement of the adjacent dura as well as an extension to the subgaleal region. No other cranial vault lesion. No cerebellar or cerebral focal lesion is noted.

Case Discussion

MRI features of a heterogeneously enhancing solitary intradiploic soft tissue mass with destruction of the inner and outer tables of the skull in a patient known for lung adenocarcinoma (pathologically proven), suggestive of skull metastasis.

The patient underwent an ultrasound-guided biopsy that confirmed the diagnosis of metastasis from lung adenocarcinoma.
