Expansile soft tissue lesion involving only the L1 vertebra is iso-hypointense on T1 and hyperintense to muscle on T2.

A correlation CT was performed. There is an expansile soft tissue lesion involving only the L1 vertebra. Prominent residual thickened trabeculae are seen giving a "mini brain" appearance.
Case Discussion
Although many vertebral lesions show low signal on t1 with high signal on t2 the "mini brain" sign is considered to be fairly characterestic of a solitary plasmacytoma. Some authors consider it typical enough to forego a biopsy and start treatment 1.
