There is well defined cystic lesion seen within the head of the right epididymis, with posterior acoustic enhancement, and associated with fine low-level internal echoes.

Case Discussion

Spermatocele is a common type of extratesticular cyst and represents cystic dilatation of tubules of efferent ductules in the head of the epididymis.

It is usually a unilocular cystic lesion but may be multilocular or septated.

It usually contains low-level internal echoes and is associated with posterior acoustic enhancement.

Differential diagnosis:

- simple epididymal head cyst

- intratesticular cysts

- ectasia of the rete testis
