Sphenoid wing meningioma

Presentation

Right proptosis.

Patient Data

Age: 40 years
Gender: Female
mri

Well-circumscribed extra-axial mass of the anterior aspect of the right middle cranial fossa with broad-based dural attachment to the greater wing of sphenoid which shows a hyperostosis. It displays an isosignal to the cortical grey matter on T1 and T2 and an iso-to-high signal on FLAIR with intense homogeneous enhancement with a dural tail sign. It extends medially to the ipsilateral cavernous sinus encasing partially the intracavernous segment of the ICA and anteriorly to the intra-and extraconal spaces through the orbital fissures, displacing an atrophied optic nerve medially and the ocular globe anteriorly with non-axial proptosis grade 3.
